How to Enroll 

Enroll in 3 Easy Steps

We can have your business signed up today!

1. Download the following TaxFreeHealth documents:

• Administrative Services Agreement
• Client Account Information
• Employee Enrollment Form – one per participating employee
• Employee Opt Out Form – if applicable

2. Follow the guidelines found in Group Sign Up Procedure to correctly fill in the enrollment documents

3. Mail the enrollment documents and cheque* to:

*The one-time only Set-Up Fee of $250.00 plus $25.00 per participating employee qualifies as a business deduction! You have no more annual fees or annoying cheque processing fees to worry about!

Upon receipt of the completed enrollment documents, SIG will mail the employer a TaxFreeHealth PHSP package containing:

• A Receipt for payment for the enrollment fee
• Copies of all signed documents
• Customized employee claim forms and supplies
• A listing of eligible expenses as per the CRA
