How to Enroll 

We can have your business signed up today!

1. Download the following Tax Free Health Inc. documents:

• Administrative Services Agreement
• Client Account Information
• Employee Enrollment Form – one per participating employee
• Employee Opt Out Form – if applicable

2. Mail the enrollment documents and cheque* to:

*The one-time only Set-Up Fee of $250.00 plus $25.00 per participating employee qualifies as a business deduction!

Upon receipt of the completed enrollment documents, the employer will be mailed a Tax Free Health Inc. PHSP package containing:

• A receipt for the enrollment fee payment
• Copies of all signed documents
• Claim forms and supplies
• A listing of eligible expenses as per Canada Revenue Agency
